I need to reschedule my appointment

We would appreciate a 24-hour notice in order to avoid a $50 penalty.

What insurance do you participate with?

While we participate with Delta Dental Premier, Delta Dental Advantage, MetLife, Aetna, United Concordia, Horizon Blue Cross Blue Shield, and Cigna, we would work with other insurance such as Guardian and United Health Care.

Do you accept Medicaid?

No, we do not accept Medicaid at this time.

What do we need to become a patient?

If you have x-rays taken within one year, please request a copy to be forwarded to us.

If I have dental insurance, do I have to pay anything?

Depending upon your dental plan, there is usually a co-payment. To be sure of your benefits, please contact the Human Resource department of your company.

Do you accept children in your practice?

We would be happy to see children 3 years and older.
